Churchville Veterinary Clinic is pleased to provide you with a variety of forms that can simplify the process of caring for your pet.

HOSPITAL POLICIES

Our doctor’s visits are by appointment only. This enables us to serve you better and will limit your waiting time. Our appointments are offered as follows:

  • Wellness Visits

  • Illness/Injury Visits

  • Drop Off Appointments

  • House Calls

 If you prefer to see a particular doctor, please designate that at the time you make your appointment.

We ask that all pets enter our clinic on a leash or in a carrier. This is for their protection as well as that of our clients. For the safety of your pet and others, we also ask that all patients are up to date on their vaccinations.

We are excited to offer online booking to schedule appointments with one of our doctors at your convenience—any time day or night! Please use the “Schedule An Appointment” link in the top banner of our page!!!

Surgical patients are typically admitted between 7:30 – 8:00 am on the day of surgery. If you cannot make this time, you also have the option of dropping your pet off the night before at no extra charge! You may call at any time to check in on your pet, and our staff will be happy to provide you with an update. Inquiries regarding animals hospitalized overnight should be made after 9:00am to enable our staff to evaluate them for any changes in their condition. In case of an unexpected change in your pet’s condition, the staff will contact you as soon as possible.

Our doctors and staff will gladly discuss how your pet is doing and answer any questions you may have. Please be sure to leave a phone number where you may be reached at all times!

Payment

All of our hospitals strive to provide you and your pet with the best care possible. In order to allow us to continue to provide a high level of care for all of our patients, we do request that payment be received when services are rendered. We accept cash, personal checks, MasterCard, Visa, American Express, and Discover. We also accept CareCredit for bills that are greater than $300. An estimate of costs may be provided upon request.

CANCELLATION/NO SHOW POLICY

We, at Churchville Veterinary Clinic understand that sometimes you need to cancel or reschedule your appointment. However, when you do not call-in advance to cancel your appointment, you may be preventing another pet from getting much needed treatment.

We kindly ask that you give us at least 24 hours’ notice if you need to cancel or reschedule your appointment. If you realize you will not be able to make your scheduled appointment, we ask that you call the office at (410)838-0085 to let us know. Unfortunately, if you do not do this, it counts as a “no show”

If you no show or cancel an appointment without 24 hour notice more than two times, you may be required to pay a deposit (office visit fee) when scheduling your next appointment.

This deposit will be applied to the total costs of your next appointment.

If a client no shows without cancelling a scheduled appointment – will be required to pay a non-refundable $25 no show fee and prepay the

$69 exam fee to schedule another appointment which will be put towards that visit.
